Congenital Heart Disease Young Adult Transition Program

The UCSF Congenital Heart Disease Young Adult Transition Program prepares teens and young adults with heart disease for the transition from pediatric care to adult medical care. We're focused on helping them gain the knowledge and self-care skills they'll need to keep themselves well as they grow into adult roles and responsibilities.

Adolescence is often a trying time, and chronic illness can make things even harder. Supported by our program, patients and families learn to navigate this phase and move down the road to independence.

Over the course of several years, we work closely with our patients to identify their unique strengths and the challenges they may face. Our multidisciplinary approach addresses medical knowledge and self-care practices, vocational and educational planning, insurance, self-advocacy skills and healthy decision-making.

Our team collaborates with the Adult Congenital Heart Disease Clinic and the Child and Adolescent Chronic Illness Center, as well as UCSF primary care providers, to make sure our patients receive the most appropriate care in adulthood. For patients who can't manage their own care independently, we identify the best resources for accessing personalized adult medical care.
